How are lost rushing yards recorded?

Lost rushing yards reduce the runner's rushing total. A carry can therefore increase the attempt count while lowering total yards and yards per carry.

For example, runs of 8, minus 3, and 6 yards total 11 yards on three attempts. Do not subtract the 3-yard loss again from that net total. NFL sack losses belong to the passing category rather than ordinary rushing losses.
